TITLE OF PROJECT
Enhancing and Assessing Teamwork through a Student Teamwork Guide
The purpose of this research is to investigate the impact of the Student Teamwork Guide on current UTA students' knowledge and understanding of teamwork. We appreciate you taking the time to contribute to this project. Your responses are completely voluntary and all data will be anonymized for presentations and publications, meaning that your responses will not be associated with your name. Only the research team at the University of Texas at Arlington will have access to the responses and we will make every effort to keep your data confidential. There are no perceived risks or discomforts for participating in this research study, but should you experience any discomfort, please inform us. You may end the survey at any time. We anticipate that the session will take up to 10 minutes. We will also collect data during the semester through a course reflection exercise. Your responses to surveys and the reflection will not impact your academic standing in any way. There are no direct benefits for participating, but your responses will improve understanding in a critical area of educational practice.
If you have any questions about the research, contact the principal investigator, Andrew M. Clark, by telephone at 817-272-3054 or by email at firstname.lastname@example.org. Any questions you may have about your rights as a research subject or a research-related injury may be directed to the Office of Research Administration; Regulatory Services at 817-272-2105 or email@example.com. The Institutional Review Board at UTA has reviewed and approved this study and the information within this consent form. If in the unlikely event it becomes necessary for the Institutional Review Board to review your research records, the University of Texas at Arlington will protect the confidentiality of those records to the extent permitted by law.
