I consent to the College of Education's disclosure of my academic records pertaining to meeting requirements of the program and state certification. This may include, but is not limited to practica experiences, exam scores, TEA ID number, etc.
Texas HB 1508 requires programs to notify all applicants that a felony conviction may make me ineligible for certification even upon program completion. The law requires that this information be provided without regard to whether or not the person has been convicted of a criminal offense. I may review current TEA criminal background check guidelines. I also have the option to request a preliminary criminal history background check from TEA to provide a criminal history evaluation letter: www.tea.texas.gov/Texas_Educator/Investigations. It is possible that some school districts will not permit individuals with misdemeanor or felony convictions to complete fieldwork on their campus.

All communication from the Certification Office will be through my UTA email account. Should I meet program admission requirements, I will receive a formal offer of admission. I understand the official offer must be accepted by the date listed in the offer to complete the admission process.
I understand I will be responsible for a $35 technology fee required by TEA 19 TAC 229.9 through my UTA student account. The charge is not immediate, but it should post within the first year of my program admission. The fee is a one-time, non-refundable charge. Therefore, if I discontinue the program and request to be readmitted to UTA or another program in the future, I will incur an additional fee.

The final authority to award teacher certification remains with the State Board for Educator Certification (SBEC) and the Texas Education Agency (TEA). Please note that a valid Social Security Number is required to complete the fingerprinting and certification process.
