When filling out a BRIDGE Alert referral form, provide clear, factual details about your concern. Include what you observed or what the student shared, when it occurred, and how it’s impacting the student’s well-being or academics. Note any actions you’ve already taken (such as speaking with the student or referring them to another office). Avoid assumptions or diagnoses—just describe the situation as objectively as possible. Be sure to include your contact information in case the BRIDGE team needs clarification.
