Study Title:  "Teen Sexual/Reproductive Health Knowledge, Attitudes, and Behaviors in Dallas County" [Screening Survey]

My name is Dr. Kyrah Brown and I am an assistant professor at the University of Texas at Arlington. My team and I are recruiting teenagers between 16 and 19 years old to participate in this research study. The purpose of this study is to understand what types of teen sexual and reproductive health information young people have seen in Dallas County recently and to understand teens’ knowledge and behaviors related to sexual and reproductive health, abstinence, and talking with parents/guardians about sexual/reproductive health.

This study has been reviewed and approved by an Institutional Review Board (IRB) at the University of Texas at Arlington. An IRB is a group of professionals who review research studies to make sure the rights and welfare of research participants are protected. The most important right for potential research participants is informed consent.

For this research study, please take time to read the information in this consent form and ask questions about anything that you do not fully understand before making a decision to participate. If you don’t want to be in this study, you do not have to. Remember, being in this study is up to you and no one will be mad or upset, even if you change your mind later and want to stop before you are finished.  That is okay.

What am I being asked to do?
If you do not want to be in this study, you do not have to be. If you say yes to being in this study, this is a list of things you will be asked to do:
Read this consent form which will give you an understanding of what this study is about. Contact the researcher if you have questions or talk with a parent/guardian if you need help deciding whether to participate.Complete the contact form. The research team will contact you to verify you as a human and provide you with a survey link. The research team reserves the right to withdraw ineligible/contaminated responses without the respondent’ permission. By agreeing to participate in this study you are agreeing to be contacted by the research team.Complete a brief online survey that includes questions about your knowledge and behaviors related to sexual/reproductive health, abstinence or safe sex, and talking with parents/guardians about sexual/reproductive health. The survey takes 6 minutes to complete. 

Do I need parent/guardian permission to participate in this research?
You are encouraged to talk with your parent, or others, about this study if it makes you feel more comfortable and confident about whether to participate. If you are 16 or 17 years old, you do not need to have your parent/guardian give permission for you to do the survey.

 How does this research benefit me?
This research study will not benefit you directly. But, it will help Dallas County organizations understand how to improve their programs to better serve teens and their families; and to prevent unintended teen pregnancy and poor health outcomes (e.g., sexually transmitted infections [STIs]).

What risks or discomforts does this research have?
This research study is not expected to pose any additional risks beyond what you would normally experience in your daily life. Some questions on this survey may make you feel uncomfortable or embarrassed. For example, some questions ask about whether or not you have ever had sex and whether your parents/guardians have talked with you about sex or sexual health topics. If you decide to participate, you can skip any question that makes you feel uncomfortable. You will not get in trouble for changing your mind about participating. This study does not ask about sexual abuse.

What do I get for participating?
Participants who complete the survey will receive a $10.00 e-gift card. You will still receive the $10 e-gift card if you skip questions that make you feel uncomfortable, but you must make it to the end of the survey. Your contact form and survey will be connected to determine authenticity, but your contact information will be permanently deleted at the end of the study.

How will my information be protected?
The survey is anonymous which means it does not have your name or any other personal information on it so your answers will remain private. It will not be possible to connect what you say in the survey to your name, and no one at your home or school will see your answers. The research team is committed to protecting your rights and privacy as a research subject.  We may publish or present the results, but your name will not be used. While absolute confidentiality cannot be guaranteed, the research team will make every effort to protect the confidentiality of your records as described here and to the extent permitted by law.
